ACEI Angioedema Question - From Daman Langguth

The question was whether there were any reports of fatal angioedema related to ACE inhibitors.
Theo was trying to get an idea, much like a Calman chart, of ball-park estimates for death risk (apparently higher in African populations?[Dean DE, etal, J Forensic Sci. 2001;46(5):1239]).
Response from Daman Langguth: "No, though have had one pt get a percutaneous tracheostomy (not with my pen).  I think there may be some ascertainment bias as those who die probably do so remotely or at home perhaps in their sleep?? - Daman"
